چکیده

A 43-year-old man with type 2 diabetes mellitus was referred for cardiovascular risk assessment 2 years ago. Other risk factors for coronary artery disease included hypercholesterolemia, smoking, and a family history of coronary artery disease. The patient was asymptomatic. Physical examination was unremarkable; his body mass index was 22 kg/m and his blood pressure was 125/80 mm Hg. Hypercholesterolemia was well regulated with statin administration (total cholesterol 3.52 mmol/L, low-density lipoprotein cholesterol 2.23 mmol/L, high-density lipoprotein cholesterol 0.94 mmol/L). ECG showed normal sinus rhythm and no additional abnormalities (Figure 1). Myocardial perfusion imaging was performed with gated single-photon emission computed tomography (SPECT) using technetium-99m sestamibi (500 MBq). Pharmacological stress was induced with adenosine; the stress ECG showed no abnormalities. On gated SPECT, left ventricular ejection fraction was 68% during stress as well as at rest; myocardial perfusion imaging revealed no perfusion abnormalities during stress or at rest (Figure 2). In addition, no coronary calcium was observed during calcium scoring (total coronary calcium score 0). However, contrast-enhanced multislice computed tomographic coronary angiography revealed the presence of diffuse nonobstructive ( 50% luminal narrowing), noncalcified plaques, as illustrated in Figure 3. Risk factor modification was advised (exercise, stop smoking) and aggressive medical therapy was initiated (administration of aspirin, statins, and an ACE inhibitor).

Introducing the practical guideline for Diabetes and Ramadan, developed by international Diabetes federation in collaboration with Diabetes and Ramadan International Alliance, 2016

Considering the fact that many Muslims with diabetes prefer to fast in spite of inhibitory advises and to answer several conflicts and controversies regarding diabetes and Ramadan, IDF in collaboration with Diabetes and Ramadan (DAR) International in Alliance decided to develop a comprehensive guideline to be used by health care professionals. This guideline was then developed on April 2016 by ...

Risk Stratification of Hemodialysis Patients with Protein-Energy Wasting Using Hand Grip Strength and Malnutrition-Inflammation Score: are Two Indices Better than One?

Background: We aimed to detect whether risk stratification of hemodialysis (HD) patients with a combination of both malnutrition-inflammation score (MIS) and hand grip strength (HGS) indices identified more precisely patients at increased risk of protein-energy wasting (PEW). Methods: This was a deductive-analytical cross-sectional study. We determined the HGS...

The potential of myocardial perfusion scintigraphy for risk stratification of asymptomatic patients with type 2 diabetes.

Patients with diabetes, in particular patients with type 2 diabetes, are at a 2- to 4-fold higher risk of cardiovascular mortality compared with their nondiabetic peers. Patients with diabetes are also more likely to have silent ischemia and less likely to survive a myocardial infarction than nondiabetic patients. Recent studies with electron beam computed tomography (EBCT) have shown that subc...

The myocardial perfusion scintigraphy in asymptomatic diabetic patients

Introduction: Asymptomatic diabetic patients are at increased risk of cardiovascular complications. Myocardial perfusion scan may be effective in risk evaluation in this population. Methods: 106 asymptomatic diabetic patients (age: min: 37, max: 82, mean: 57.73±8.88), including 56 females (52.8%) and 50 males (47.2%) were enrolled in the study. Myocardial perfusion scintigraphy was performed b...

بررسی شیوع عفونت ادراری بدون علامت در بیماران با دیابت ملیتوس

Background and purpose: Urinary tract infection (UTI) is one of the most common and the most important clinical problems in patients with diabetes mellitus. In asymptomatic form it could cause symptomatic UTI and sepsis. There is controversy over the prevalence and risk factors of asymptomatic UTI. This study was conducted to determine the prevalence of asymptomatic UTI in diabetic patients att...

Performance of electrophysiologic study in an asymptomatic patient with type 2 intermittent Brugada syndrome: To do or not to do?

Background: Brugada syndrome (BrS) is an inherited channelopathy, which is associated with sudden cardiac death due to rapid polymorphic VT or VF. There is no definite consensus regarding the management of asymptomatic patients. Some experts advocate close follow-up; others propose the programmed stimulation for risk stratification. We aimed to evaluate the benefit of complete atrial and ventri...
